Offered in three different service levels, the Care+ program extends and incorporates therapy goals beyond the therapy gym and into a patient’s or resident’s daily activities.
- Therapist-guided restorative care and functional activities
- Routine care protocols complement skilled therapy plan of care
- Sub-acute patient experience enhanced with additional routine services
- Therapists provide thorough evaluations, participate in an interactive interdisciplinary team and use skilled and routine therapy approached to optimize outcomes
- Proprietary modules to guide therapy-infused activities
- Guided by Reliant therapy team
- Activities conducted by Restorative Nurse Aide under supervision of a Restorative Nurse
- Restorative team can be employed by the facility or Reliant but guided by therapy
- Proprietary modules to extend therapy program into daily activities
- Dedicated wellness coordinator acts as resident “concierge”
- Prescriptive programmatic activity to support body, mind and spirit
- Personalized activity navigator to tailor wellness activities to each resident
- Incorporates traditional and non-traditional senior living activities
- Programs may include music therapy, Tai Chi, pet therapy, art classes, yoga and more
